Emergency Contact:

PARENT OR GUARDIAN AUTHORIZATION AND AGREEMENT

  1. I will have my camper on time and ready to participate each day. I will not send my child to an in-person camp with a fever or other signs of illness that may affect other campers and will contact the Site Director if my camper cannot attend a session.

  2. I authorize SCR staff to seek emergency medical attention for my camper if I cannot be reached or there is insufficient time to reach me. I authorize health care professionals to provide emergency care and treatment for my child.

  3. SCR has my permission to use photographs and video images of my camper for SCR promotional purposes.

  4. I understand that neither SCR nor any of its staff or volunteers can be held responsible in the event of an accident or injury. In consideration of my child’s participation in SCR activities, I release from any claims and demands SCR and its affiliates (including the Diocese of Southern Ohio), personnel, and volunteers. For myself, my heirs, executors, and administrators I waive, release, and forever discharge any claims which my child or I may have arising out of or connected with my child’s participation in SCR activities.

  5. I understand that SCR will follow safety standards for in-person camps based on site protocols and that SCR may need to change to an online program for reasons of public health. If that occurs, I will provide wifi access and equipment needed for online participation. If SCR lends equipment for online use, I will return it to SCR in good condition at the end of camp.

  6. I pledge to spend at least 10 minutes per day talking with my child about reading and the Summer Camp Reading experience, including what my child is learning, what good things are happening, and how I can support reading.

  7. I will maintain good contact with SCR staff and work with SCR to maximize the summer experience and strive for a lifetime of achievement and enjoyment of reading.
